Do-Not-Call & Telemarketing Recordkeeping (FTC TSR + Wisconsin)
UniqueIHPC observes National Do Not Call protections for texts and calls, maintains an internal DNC list, and respects quiet hours (no calls before 8:00 AM or after 9:00 PM local time). For covered campaigns, we maintain telemarketing records for 5 years, including scripts, prerecorded messages, call detail records, caller ID authorization, consents, and dispositions. Where required, we register with DATCP (Wisconsin) and the FTC DNC Registry.
Why: In 2024, FTC amended TSR to expand recordkeeping to 5 years and strengthen B2B coverage; WI law regulates telemarketing practices and uses the national registry.
Electronic Signature & Records (E-Sign Notice)
By submitting this form electronically, you consent to receive disclosures and provide your signature electronically. We present required disclosures in an electronic format you can access and retain. You may withdraw consent at any time. We retain consent records (date/time/IP/form version) to demonstrate compliance.
HIPPA & Marketing Communications
UniqueIHPC does not use or disclose Protected Health Information (PHI) for marketing without the patient’s HIPAA authorization, except for permitted treatment/operations communications (e.g., care coordination). We do not sell PHI or patient lists. Marketing communications based on PHI require a specific written authorization stating purpose, scope, recipients, and (if applicable) any financial remuneration involved.
Why: HIPAA defines marketing and generally requires prior authorization when PHI is used to encourage purchase/use of a product or service.

(c) honor unsubscribe requests within 10 business days without fees or additional steps; and
